Moriarty doesn't actually say who that third bullet is for. Mycroft is family. Family and friends are different to most people. So of course Sherlock assumes it will be a non-family member. But it may well not have been Lestrade who was the target. I doubt Moriarty thought Sherlock would assume Mycroft was a friend, and considering that Mycroft was feeding Moriarty information about his brother, I doubt Moriarty would think of Mycroft as one of Sherlock's friends and an outside observer might make the mistake of thinking they do not love one another.
OMG! Your insights really wish I was able to find these insights. Sherlock would jump for his brother but it wasn't one of the choices.
